The Lucas County Coroner has ruled in the following deaths:

Michael Taylor, 44, May 21, at Mercy Health St. Vincent Medical Center. Accidental, blunt head trauma and complications sustained from being struck by vehicle.

Damen Teeters, 44, of Toledo, March 18, at home. Accidental, combined fentanyl and cocaine toxicity.

Bernard Thoma, 84, of Monroe, March 29, at Mercy Health St. Vincent Medical Center. Accidental, blunt head trauma with brain contusions; deceased fell.

Richard Thomas, 46, of Toledo, April 3, at Mercy Health St. Vincent Medical Center. Accidental, combined drug toxicity (codeine, fentanyl, and oxycodone).

Rosalie Thomas, 22 months, May 22, at Hospice of Northwest Ohio on South Detroit. Accidental, ischemic encephalopathy due to craniocerebral injuries due to motor vehicle accident.

Mary Beth Thomas, 67, of Norwalk, Ohio, Aug. 29, at Ebeid Hospice on Harroun, Sylvania. Accidental, chronic obstructive pulmonary disease exacerbated by right hip fracture; deceased fell.

Terrance Thompson, 30, of Toledo, Feb. 16, 2023, at home. Accidental, combined drug toxicity fentanyl and cocaine.

Brian Thompson, 43, of Toledo, March 29, at home. Accidental, cocaine and fentanyl toxicity.

Dominic Vasquez, 24, of Toledo, Aug. 30, in 5500 block of San Paulo. Suicide, deceased hanged himself.

Roberto Villarreal, 64, of Oregon, March 19, at Hospice of Northwest Ohio on South Detroit. Accidental, complication of ischemic cerebral infarct; deceased fell.

Thomas Wilson, 80, of Sylvania, June 7, at Elizabeth Scott Nursing Home in Maumee. Accidental, complications of blunt trauma with left hip fracture.

Billy Winfree, 51, of Toledo, March 26, in 2900 block of Lawrence. Accidental, combined drug toxicity (fentanyl and ethanol).

Mark Winzeler, 70, of Whitehouse, July 27, at Mercy Health St. Vincent Medical Center. Accidental, blunt trauma with subdural hemorrhage, brain contusions and skull fractures; deceased fell.

Gary Wolfe, 66, of Lambertville, Oct. 28, 2022, at ProMedica Toledo Hospital. Accidental, hemoperitoneum 900CC due to lacerated liver due to robotic surgery for colectomy.

San Marcos Restaurant, 2060 W. Laskey, inspected Feb. 13. A bowl of rice was inside the hand-washing sink by the four-bay sink. Chef base and the cake-display cooler were too warm. Chemical spray bottles had no labels.

Red Crab Juicy Seafood, 4941-4945 Dorr, inspected Feb. 13. The hand-washing sink by the dish machine had dishes in it and was blocked by a trash can. Raw shell eggs were stored incorrectly above vegetables in the walk-in cooler. Chicken in the walk-in freezer was uncovered. Quaternary ammonium sanitizer in a spray bottle was too strong. Raw shell eggs and other time/​temperature controlled foods in the prep-top cooler were too warm. Gnats were in the dish area.

Glass City Child Care, 3006 Monroe, inspected Feb. 13. Open cans/​jars of salsa, spaghetti, and various milk alternatives were held past their marked expiration dates.

Dorr Street Cafe, 5243 Dorr, inspected Feb. 12. The pizza oven was dirty with food debris. The ice machine’s slide was dirty with black buildup.

Yang’s Gourmet House, 4446 Heatherdowns, inspected Feb. 14. Raw meats were stored incorrectly above cooked rice in the walk-in cooler.

The Black Pearl, 4630 Heatherdowns, inspected Feb. 14. The person-in-charge used the hand-washing sink by the ice machine improperly to make a corn starch slurry. The can opener’s blade was dirty with food debris. The ice scoop was cracked and missing a piece. Drain plumbing for the server station ice bin and the three-bay prep sink by the ice machine lacked proper air gaps to prevent sewage backflow.

McDonald’s, 36 N. Reynolds, inspected Feb. 14. The Traulsen stand-up refrigerator and lettuce inside the cooler were too warm. Gravy sitting on the counter above the microwaves holding at an unsafe temperature. Cheese sitting out on the line had no time stamp.

Crab Island Seafood Market, 5151 Hill, inspected Feb. 14. No person-in-charge present in food facility during inspection. Bins of potatoes holding in water sitting under shelving in the walk-in cooler were not clean. Raw fish and other raw time/​temperature controlled foods were stored improperly above ready-to-eat foods. The inside of the oven, the hand-washing sink, paper-towel dispenser, and fry cutter were dirty. No sanitizer was set up. Raw fish and other seafood were too warm.

Chili’s Grill & Bar, 4801 Talmadge, inspected Feb. 14. Stainless steel covers for the 3 compartment sink basins all damaged in the corners and are no longer easily cleanable.

Boil Shack, 4038 Talmadge, inspected Feb. 14. Many cardboard boxes blocked access to the kitchen hand-washing sink. Raw shrimp were stored improperly above cooked chicken in the prep-top cooler. Raw shrimp were too warm. An employee beverage was stored improperly in the ice machine’s ice.

Toledo Museum of Art, 2445 Monroe, inspected Feb. 15. The dish machine was not dispensing chlorine sanitizer when required. Hot wells for cheese sauce and vegan cheese sauce were not functioning correctly and the sauces were too cool. Milk crates were used improperly for storage in the walk-in cooler.

Rose Thai, 5333 Monroe, inspected Feb. 15. Food in the walk-in cooler lacked covers. Tofu sitting out was too warm. A spray bottle the person-in-charge said contained bleach solution was unlabeled.

Nick & Jimmy’s Bar & Grill, 4956 Monroe, inspected Feb. 15. Food inside the beer walk-in cooler lacked covers. A spray bottle stored on top of a chest freezer had no label. Small flying insects were present at both bars.

Merit House, 4645 Lewis, inspected Feb. 15. The ice machine was dirty inside with buildup. Numerous gnats entered the beverage area from the drain.

Kira Japanese Steakhouse, 3324 Secor, inspected Feb. 14. A bowl of raw shrimp was stored in direct contact with cut onions in the walk-in cooler. Foods including cooked noodles and cut vegetables in the walk-in cooler lacked covers. The dish machine was not dispensing sanitizer. Soy sauce made in the facility lacked date marks.

Golden Dragon, 5624 Secor, inspected Feb. 15. Food employee(s) did not wash their hands when required. The dish machine was not sanitizing properly. Bean sprouts sitting out were too warm.

Beirut Restaurant, 4082 Monroe, inspected Feb. 12. The hand-washing sink near the front counter was not easily accessible. The main meat grinder was dirty with buildup. The parsley chopper was dirty with green residue. Bread/​dough equipment had buildup from the previous day. The three-bay sink, pop gun, and gun holster were dirty. Too much food was being prepared at once, resulting in food being left out of refrigeration too long. The dish machine did not get hot enough during the wash cycle.

Arnies Bar & Grill, 2633 W. Bancroft, inspected Feb. 15. Quaternary ammonium sanitizer in buckets was too strong.

South Side Liquor Box, 754 Western, inspected Feb. 16. No person in charge qualified in food safety was present during inspection. No sanitizer was set up during food handling. The pop nozzle was dirty.
