Scott Baldwin, 47, of Toledo, Nov. 9, 2022, at home. Suicide, self-inflicted gunshot wound to the head.

Wing Depot, 5318 Heatherdowns, inspected Oct. 17. A container of cut potatoes holding in liquid in the walk-in cooler was too warm. An open container of barbecue sauce and portioned cups were left out at room temperature.

Turkey Up Toledo, 3200 N. Holland-Sylvania, inspected Oct. 17. The person-in-charge could not show knowledge of relationship between safe food temperatures and prevention of foodborne illness. Raw shell eggs and raw fish were stored improperly above ready-to-eat foods. Macaroni and cheese, stuffing, and side dishes in hot holding had been reheated improperly. Sausage left out of refrigeration was too warm. Thawed, cooked turkey legs left out at room temperature were too warm. Chemical containers lacked labels. Consumer-grade insect spray was used improperly in the facility.

S&G, 1301 W. Sylvania, inspected Oct. 17. Stock blocked access to the hand-washing sink. Employees did not wash their hands. No sanitizer or test strips were available and no drain stoppers were available for the three-bay sink. The mop sink was full of damaged merchandise returns. An open, partially used bag of cheese sauce sat in a cabinet beneath the customer self-service counter. The walk-in cooler’s ambient air temperature was 60 degrees, requiring all temperature sensitive foods inside to be discarded including five cases of raw shell eggs, five cases of bagged creamer, a case of yogurt, and two cases of breakfast burritos. Thawed sandwiches in the deli display case lacked date marks.

St. John’s Jesuit High School, 5901 Airport, inspected Oct. 17. Bulk lunch meat packages were open without required date marks.

Neighborhood News, 5001 Monroe, inspected Oct. 17. No person-in-charge qualified in food safety was present during inspection. Beverage cans were stored improperly in the hand-washing sink’s basin. The cappuccino machine’s interior components were dirty with food buildup. Consumer-grade inspect spray was improperly present in the facility. CapriSun pouches, Haagen Dazs ice cream bars, and Oreo sandwiches offered for sale did not have labeling for individual sale. Bulk self-service candy containers lacked scoops.

McDonald’s, 90 Main, inspected Oct. 17. The hand-washing sink by the ice machine had an inadequate water supply.

Golden Lily, 2223 S. Reynolds, inspected Oct. 17. No sanitizer bucket was set up in the kitchen. Chicken and egg rolls were too warm after being taken out of cold-holding.

Foundation Park Care Center, 1621 S. Byrne, inspected Oct. 17. Controlling pests. Presence of live insects, rodents, and other pests. Observed gnats around the dish machine area in the back of the facility. Live pest shall be controlled.

Circle K, 3819 Haverhill, inspected Oct. 17. Flies, bees, and gnats got in through a wide-open front area during construction and rat feces were in the back furnace room.

Boston Market, 3808 Secor, inspected Oct. 17. The wall-mounted potato slicer was dirty with a black mold-like substance. A half-chicken in the Alto Sham was holding at an unsafe temperature. A open exterior door allowed flies inside.

Black Rock Bar & Grill, 5001 Monroe, inspected Oct. 17. High number of critical and repeat violations indicated lack of supervision by person-in-charge qualified in food safety. The back prep area’s hand-washing sink was blocked and did not work. Containers of baked sweet potatoes and ribs in the keg walk-in cooler lacked covers. A washed lettuce bin had standing water in it and remained dirty. The bar pop gun was dirty with buildup. The chemical dish machine at the bar did not dispense chlorine sanitizer during final rinse. The ice machine’s interior was dirty with buildup. Baked potatoes in the hot-holding cabinet were not hot enough. An opened container of bloody Mary mix on ice at the bar was too warm. Repeat violation. Volcano sauce in the prep-top cooler was too warm. Portioned cups of sour cream in the expo cooler were too warm. Andouille sausage double-panned in the prep-top cooler was too warm. Partially cooked bacon and a bag of Parmesan cheese labeled Keep Refrigerated were left out at room temperature. A five-gallon container of chowder had no labeling or date mark. Chemicals were stored improperly on a food-prep table and above clean dishware. Flying insects were present in the bar area and kitchen.
