Rally’s, 1647 W. Laskey, inspected Aug. 9. Food was not properly reheated before being placed in the steam table.
Pizza Hut, 1654 Ralston, inspected Aug. 9. The cinnamon sugar dispenser, cheese shakers, and lids were dirty. Food debris, sauce bottle lids, and funnels were on the dish storage rack. Lids were cracked and broken.
Molly’s Grocery Store of Toledo, 969 Rogers, inspected Aug. 9. Raw shell eggs in the display cooler were too warm. The bathroom sink had no hot water. The mop sink had no backflow prevention device or air gap to prevent back flow.
Wesley’s, 1201 Adams, inspected Aug. 9. The hand-washing sink was used improperly as a dump sink. A working chemical bottle had no label.
Scooter’s Coffee, 4023 Talmadge, inspected Aug. 10. Two cans of whipped topping were left out at an unsafe temperature. Quaternary ammonium sanitizer in the three-bay sink and wipe bucket was too strong. Chemical spray bottles hung improperly from a cart storing food.
Reynolds Garden Cafe, 1240 S. Reynolds, inspected Aug. 10. No sanitizer was set up during food handling. Tomatoes, cut lettuce, and other time/temperature sensitive foods were too warm.
Pizza Hut, 3425 Secor, inspected Aug. 10. The chemical dish machine failed to dispense chlorine sanitizer during final rinse.
Penn Station East Coast Subs, 4798 Monroe, inspected Aug. 10. The fountain beverage station’s nozzles were dirty with buildup. A cell phone was stored improperly on the slicer’s food-contact surface. Turkey and ham in the under-counter, two-door cooler were too warm.
Papa’s Tavern 2.0, 1328 Liberty, inspected Aug. 10. Controlling pests. Flies were present in the kitchen and the bar area.
Hannon’s Block Restaurant, 619 Monroe, inspected Aug. 10. The ice machine’s chute was dirty with pink slime buildup. Corned beef, raw bacon, and cheese inside the grill cooler, and raw eggs and sliced cherry tomatoes elsewhere were cold holding at unsafe temperatures.
Graze Shared Kitchen, 1734 W. Laskey, inspected Aug. 10. The Hobart mixer and the pop nozzles’ insides were dirty with buildup. The Olga seasoning container and both dough sheeters were filthy.
Domo Sushi, 6725 W. Central, inspected Aug. 10. The cutting board was dirty. Live gnats were present.
Berger’s Olde Tyme, 1742 W. Laskey, inspected Aug. 10. Opened hot dogs had no date mark. Bologna was held past its marked expiration date.
Anchor Inn, 4950 Suder, inspected Aug. 10. The shield inside the ice machine next to the walk-in cooler had excessive black buildup. The patio bar’s pop gun was dirty inside with pink buildup.
7-Eleven, 3749 Upton, inspected Aug. 10. The person-in-charge failed to ensure employees are property trained in food safety. No one was on site at time of inspection with proper food safety training. The person-in-charge failed to ensure employees maintain safe food holding temperatures. The under-counter cooler below the microwave failed to maintain proper refrigeration. The person-in-charge failed to ensure employees wash their hands properly. Hot running water was not available at the hand-washing sinks for the counter food service area or the rest room. The person-in-charge failed to ensure wares are sanitized after washing. Dirty water from the previous night was in the three-bay sink and sanitizer wipe cloth bucket and sanitizer was not set up. No sanitizer test kit was available in the facility. Employees were not verifiably informed of duty to report health conditions relevant to food safety. The pizza wheel cutter and spatula were encased in old, dried food. All food rollers were dirty with food debris and grease. Opened time/temperature sensitive foods lacked date marks. Flies swarmed around old rags and used cardboard boxes by the rear door. A large amount of rat feces accumulated under the back stock room’s dry storage rack. The facility was infested with gnats.
