The Lucas County Coroner has ruled in the following deaths:

Rashad Ali, 32, of Toledo, Nov. 23. Accidental, multiple blunt-force injuries sustained from accident.

Zacharias Barden, 41, of Toledo, Nov. 8, at home. Suicide, multiple stab wounds; deceased stabbed himself.

Anjelica Church, 22, of Adrian, May 29, at ProMedica Toledo Hospital. Accidental, recent fentanyl usage.

Adelaide Curry, 80, of Toledo, Nov. 26, at home. Accidental, pancreatitis with pancreatic resection; deceased fell.

Marlene Dembowski, 89, of Toledo, Nov. 26, at Hospice of Northwest Ohio on South Detroit. Accidental, blunt trauma with right femur (hip) fracture; deceased fell.

Khalid Denton, 29, of Toledo, Nov. 28, at Mercy Health St. Vincent Medical Center. Accidental, blunt head trauma sustained from vehicle accident.

Joseph Drusbacky, 82, of Oak Harbor, Ohio, Aug. 8, at Mercy Health St. Vincent Medical Center. Accidental, subdural hematoma due to a fall; deceased fell at home.

Joseph Giacoppo, 55, of New Hampshire, May 11, in the 5300 block of Heatherdowns. Accidental, fentanyl toxicity and cocaine.

Daevon Higgs, 22, of Toledo, Aug. 5, at Mercy Health St. Vincent Medical Center. Homicide, multiple gunshot wounds.

Charles House, 59, of Toledo, June 30, at home. Accidental, probable oxycodone toxicity.

Darcea Jackson, 62, of Toledo, Nov. 11, at Hospice of Northwest Ohio on South Detroit. Accidental, anoxic encephalopathy due to probable cocaine toxicity.

Ronald Jarrells, Jr., 36, of Toledo, June 29, at home. Accidental, fentanyl toxicity.

Patricia Lemle, 90, of Holland, Nov. 5, at ProMedica Toledo Hospital. Accidental, bilateral bronchopneumonia due to blunt-force head injuries; deceased fell at home.

Dolores Martinez, Jr., 84, of Oregon, Nov. 16, at ProMedica Toledo Hospital. Accidental, multiple medical co-morbidities complicated by a fall with a subdural hematoma; deceased fell at home.

Anthony Pandi, 33, of Toledo, Aug. 1, in the 300 block of South Reynolds. Suicide, self-inflicted gunshot to the chest.

Ashley Parker, 28, of Toledo, Aug. 14, at ProMedica Toledo Hospital. Accidental, multiple blunt-force injuries sustained from vehicle accident.

TJ Roberts, 81, of Toledo, Nov. 16, at Hospice of Northwest Ohio on South Detroit. Accidental, complications of quadriplegia; deceased fell down stairs 30 years ago.

Steven Robinson, 68, of Swanton, July 29. Accidental, blunt head trauma sustained from vehicle accident.

Chester Rutkowski, 85, of Lambertville, Nov. 20 at Hospice of Northwest Ohio on South Detroit. Accidental, chronic obstructive pulmonary disease; deceased fell at home.

Tori Saunders, 27, of Toledo, June 14, at home. Accidental, probable cocaine and fentanyl toxicity.

Valerie Sayers, 71, of Oregon, Aug. 17, in the 2500 block of Hollydale, Oregon. Suicide, ligature self-strangulation; deceased tightened zip ties around neck.

Wanda Siebenaller, 91, of Fostoria, Nov. 7, at ProMedica Toledo Hospital. Accidental, saddle pulmonary thromboembolis due to disrupted organized thrombus of left leg deep vein due to blunt-force injury of the left leg.

Konrads Voits, of Oregon, Aug. 13, at Mercy Health St. Vincent Medical Center. Suicide, self-inflicted gunshot wound to the head.

Anthony Westmeyer, 49, of Toledo, Aug. 2, at Hospice of Northwest Ohio on South Detroit. Accidental, medical complications (aspiration pneumonia, and staph epidermidis bacteremia due to right femur fracture.

Maria Williams, 87, of Maumee, Nov. 19, at Hospice of Northwest Ohio on South Detroit. Accidental, medical complications of left hip fracture; deceased fell at home.

Tomazz Wilson, Jr., 25, of Toledo, Nov. 15, on Corduroy at North Wynn, Oregon. Accidental, multiple blunt-force injuries sustained from vehicle accident.

Ervin Wisniewski, 93, of Sylvania, Nov. 10, in 4100 block of King, Sylvania. Accidental, subdural hematoma; deceased fell.

Jack Wittler, 32, Nov, 17, in the 2000 block of Parkside. Suicide; self-inflicted intraoral shotgun wound.

Mark Wysinger, 32, of Toledo, Nov. 5, at Hospice of Northwest Ohio on South Detroit. Homicide, bilateral bronchopneumonia due to gunshot wound to the head.

Walmart, 2925 Glendale, inspected June 12. Quaternary ammonium sanitizer in the produce department was too weak. The prep and three-bay sinks in the produce room were dirty with debris. Chicken tenders were hot-holding at an unsafe temperature. Gnats were present near the produce room’s three-bay sink.

S&G, 1301 W. Sylvania, inspected June 12. Stored items blocked access to the hand-washing sink. Half of the open-air display cooler was not working and foods inside were too warm. Hoses attached to the cleaner system’s spigots behind the three-bay sink lacked required backflow prevention devices.

Street Tacos Mexico, 1010 Broadway, inspected June 12. Food truck employees did not wash hands because no hot water was available. Cords were stored improperly in the hand-washing sink. The three-bay sink blocked access to the hand-washing sink. Foods were obtained from a source not licensed for wholesale. No sanitizer bucket was set up for wipe cloths. Corn in a household crockpot was holding at an unsafe temperature. A gallon jug of mayonnaise was left out at room temperature and a crust formed on it. Raw chorizo in the drawer cooler was too warm.

Roman’s Deli, 526 Jackson, inspected June 12. Cole slaw and desserts lacked date marks.

Palacio Maya, 5125 Monroe, inspected June 12. No person-in-charge qualified in food safety was present during inspection. A food handler chopped a ready-to-eat onion without wearing gloves. An apron was stored in the cook line’s hand-washing sink and a water bottle and wet rag were in the back hand-washing sink by the ice machine. Foods in the walk-in cooler, walk-in freezer, and a bin on the cook line lacked proper covers. Raw shell eggs were stored improperly above cucumbers in the produce walk-in cooler. The bar pop gun and its holder were dirty with buildup. Enchilada sauce in three five-gallon containers in the walk-in cooler had been cooled improperly and was too warm.

Kabob It, 5001 Monroe, inspected June 12. Cleanliness of equipment food contact surfaces and utensils. Equipment food contact surfaces or utensils are unclean. Observed build up on the panini press, inside the ice machine and on the iced tea dispenser spout. Thoroughly clean and sanitize all food contact surfaces. Cooling - temperature and time control. Improper cooling of time/​temperature controlled for safety food. Observed lentil soup in the walk-in cooler that was 103 degrees and had been cooling for at least five hours according to the person in charge. All time/​temperature controlled for safety food shall be cooled from 135 to 70 degrees within two hours and then from 70 to 41 degrees within the following four hours to prevent the growth of bacteria. Discard the soup as it did not reach 70 degrees within two hours. Time/​temperature controlled for safety food - cold holding. Time/​temperature controlled for safety foods not being cold held at the proper temperature. The two-door cooler is holding time/​temperature controlled for safety foods such as tzatziki at 46 degrees and chicken Babob’s at 44 degrees. All time/​temperature controlled for safety food shall be held cold at 41 degrees or less to prevent the growth of bacteria. Discard the out of temperature food. Ready-to-eat, time/​temperature controlled for safety food - date marking. Refrigerated, ready-to-eat, time/​temperature controlled for safety foods not properly date marked. The rice pudding containers were not properly date marked. Provide a seven day expiration date for the rice pudding to prevent the growth of bacteria. Food contact surfaces - cleanability. Food contact surfaces not easily cleanable. Observed the cutting boards are heavily scored and are no longer easily cleanable. Replace all of the cutting boards with ones that smooth and easily cleanable. Food-contact surfaces - cleanability. Food contact surfaces not easily cleanable. Observed cracked food container lids in use that are no longer easily cleanable. Discard the damaged lids and replace with new ones that are smooth and easily cleanable.

Jeannie’s Coffee Shop, One Government Center, inspected June 12. Chicken salad, ham lunch meat, and salad dressing in the two-door prep cooler were too warm. Cut fruit lacked an expiration date.

Woodlands at Sunset House, 4030 Indian, inspected June 12. The ice machine’s slide was dirty with buildup. Fish and sauce in the two-door cooler were too warm.

Sunset House - Main Kitchen, 4020 Indian, inspected June 13. The blender and mixer near the prep sink were dirty with food debris. The main kitchen’s ice machine and the ice and water dispensers in the main and satellite kitchens were dirty with buildup. Cooked peppers and onions, cheese, and lettuce were stored improperly above the prep-top cooler’s chill line and were too warm. Roast beef, corn relish, pizza sauce, sour cream, and cottage cheese in the prep-top/​under-counter cooler and cut tomatoes and cottage cheese in the satellite kitchen’s cooler were held past their marked expiration dates. Plastic container lids were cracked and broken.

Plate One, 420 Madison, inspected June 13. The meat slicer was dirty with protein buildup. A chemical bottle had no label.

McDonald’s, 1205 W. Sylvania, inspected June 13. The pop fountain nozzles by the drive-up window and in the dining room were dirty with a mold-like substance.

Circle K, 3516 Airport, inspected June 13. Raw bacon was stored improperly above ready-to-eat deli meat.

Circle K, 2969 Tremainsville, inspected June 13. Hot water supply was inadequate.

Alexis Carryout, 3519 W. Alexis, inspected June 13. Eggs were stored improperly above milk in a cooler.

Volunteers of America of Greater Ohio, 1323 Champlain, inspected June 15. The person-in-charge failed to ensure employees sanitize washed equipment and utensils. No sanitizer solution was set up in the three-bay sink or in a wipe-cloth bucket during food preparation.

Hamburger Mary’s, 26 Main, inspected June 15. A dirty knife hung in the knife holder. The ice machine’s chute was dirty with pink slime buildup.

Wendy’s, 4277 Monroe, inspected June 16. The Pattie cooler holding raw ground-beef patties and raw shell eggs in the Pattie cooler failed to maintain proper refrigeration temperature.

McDonald’s, 2130 S. Reynolds, inspected June 16. The ice machine’s slide was dirty with black buildup. Drain plumbing for the ice machine, prep sink, ice bins, soda machines, and soda machine ice machines lacked air gaps to prevent sewage backflow.

Waffle House, 5445 Monroe, inspected June 20. A container was stored improperly in the cookline hand-washing sink, and an employee improperly filled a container with water there. A chemical spray bottle hung on a rack also holding single-service food and food-contact items. Drain plumbing for the ice machine, ice bin, and soda dispenser lack air gaps to prevent sewage backflow.

The Mindful Table, 4334 W. Central, inspected June 20. Fiesta dip being delivered cold from the Perrysburg store was too warm. Quaternary ammonium sanitizer solution was too strong. Sloppy joes and lasagna in the hot-holding unit were too cool. Lasagna, enchiladas, cheesy hash browns, salads, and paella in the large display cooler were too warm. Containers of macaroni salad, lobster salad, deviled potatoes, chicken salad, tomato basil dip, mac and cheese, and fiesta dip were stored above the sushi case/​deli cooler chill line and were too warm.
